New Patient Information
For your convenience, please feel free to download, print and fill out the following forms and bring them into your first appointment with us.
First Visit: 60-minute appointment
We are always excited to meet new patients during their first visit to our office and we strive to provide our patients (and parents) with the most information possible regarding orthodontic needs. We do this by taking diagnostic digital photographs of your face, teeth and bite and then complete a thorough examination with a discussion of potential treatment options as well as an approximate treatment fee. This important 60-minute visit will give Dr. Pavlo insight into your orthodontic needs.
During the initial examination for each patient, Dr. Pavlo will address the following questions that cover the basics of orthodontic treatment. We encourage you to ask questions as well.
- Is there a condition that orthodontics can address?
- Is treatment needed now or should treatment be delayed until appropriate growth, tooth eruption or other factors have occurred?
- What treatment procedures will be used to correct the problem?
- Do any permanent teeth need to be removed?
- How long will treatment take?
- How much will it cost?
- What are my payment options?
While we can often answer these general questions about treatment during the initial examination, we will address specific areas after careful analysis of the orthodontic records. We feel it is important to analyze each patient’s specific needs because in- depth planning leads to superior results and higher patient satisfaction.
Please assist us by providing the following information at the time of your first visit:
If you have orthodontic insurance, bring your insurance card. By providing this information at the first visit, we will be able to give you an estimate of your costs.
With our lives being so busy, many patients wish to start treatment as quickly as possible. To accomplish this, we can complete the process of gathering all the remaining diagnostic information (orthodontic records) at the first visit (about 20 minutes). Orthodontic Records include digital photographs, a digital panoramic x-ray and a digital cephalometric x-ray (growth x-ray). Time will be reserved to complete this appointment during the initial visit for those patients that Dr. Pavlo feel are at the appropriate stage to begin. At the next visit, a treatment conference is held to finalize all treatment details. Once all issues have been reviewed and agreed upon, treatment can begin immediately.
Treatment Conference: 30-minute appointment
In some instances, it is preferred to divide this process into individual visits. In these cases, a separate treatment conference will be set up. This allows either the doctor additional time for consultation with the patient’s dentist or for the patient to review their treatment options.
After the orthodontic records are taken, we will then schedule the treatment conference visit to discuss treatment options, time frames and financial arrangements. We insist that our patients leave the office with a clear understanding of their specific needs, what the treatment will consist of and how long it will take. Also, we will answer any additional questions.
Regardless of the number of visits before beginning treatment, Dr. Pavlo is most concerned with satisfying your original reason for seeking treatment. Your input into the various treatment options is very important to us.
